Our Coordinators of Community Services (CCS) partner with individuals with intellectual and developmental disabilities and their families to understand what matters most to them, navigate services and resources, advocate for their needs, and support progress toward meaningful life goals.This is rewarding, relationship-centered work that also requires independence, organization, accountability, travel, timely documentation, and consistent follow-through. We want candidates to understand both the purpose and the day-to-day realities of the position before joining our team.WHAT THIS ROLE IS REALLY LIKEThis is a community-based position—not a traditional remote job.- Administrative work may be completed from a home-based workspace, but regular in-person community work is an essential part of the position.
- You will routinely travel throughout your assigned service region.
- Meetings may occur in individuals' homes, provider locations, workplaces, programs, and other community settings.
- Travel varies according to caseload needs and may occasionally involve driving up to approximately two hours one way.

- You must have reliable transportation and the ability to travel throughout your assigned region.
- Build trusting, person-centered relationships with individuals and their families or support systems.
- Learn what matters most to each person and help translate their goals into meaningful supports and services.
- Coordinate and monitor services across providers, agencies, community resources, and other support systems.
- Connect individuals and families with appropriate community, healthcare, employment, housing, educational, financial, and social resources.
- Monitor whether services are meeting the individual's identified needs and goals, and advocate for the individual when barriers to services or supports arise.
- Participate in planning meetings and collaborate with individuals, families, providers, State agencies, and community partners.
- Maintain accurate, timely, and person-centered documentation of case management activities in accordance with organizational and regulatory requirements.
- Manage an assigned caseload while meeting quality, timeliness, productivity, and compliance expectations.
- Participate in supervision, training, quality review, and professional development.
- Maintain confidentiality and protect sensitive information.

- Possess a Bachelor’s or graduate degree in social work or another health or human services field.
- Complete a supervised field experience in case management, behavioral health, or vocational rehabilitation as part of the degree or state’s recognition requirements for permitting legally authorized practice, or two (2) years of full-time equivalent experience providing direct clinical care to the consumer
- Coordinating services for people in Medicaid and/or waiver programs
- Coordinating services for people with intellectual disabilities/developmental disabilities (ID/DD)
- Social Service intake and referral services
- Coordinate services for eligible individuals as outlined in the Coordinator of Community Services COMAR, the DDA Scope of Service
- Monitoring the provision of services to participants
- Allied service delivery systems, including Medicaid, mental health, substance abuse, social services, juvenile justice, vocational rehabilitation, and corrections
- Regulations governing services for participants
- Work is performed in a combination of home-based and community settings.
- Regular travel throughout the assigned counties and service region is required.
- Community work may occur in homes, workplaces, programs, provider locations, and other public or private settings.
- Work environments may occasionally be unfamiliar, uncomfortable, or present potential health or safety concerns.
- Appropriate personal protective equipment may be required depending on the environment.
- Employees are expected to exercise professionalism, situational awareness, sound judgment, and appropriate safety practices while working independently in the community.
