Medical Social Worker

We are seeking a compassionate and dedicated Social Worker to support individuals and families by providing guidance, resources, and advocacy. The Social Worker will assess client needs, develop care plans, and coordinate services to help improve overall well-being and quality of life.

Responsibilities:

• Conduct assessments to identify client needs and challenges

• Provide emotional support, counseling, and crisis intervention when necessary

• Connect clients with community resources and support services

• Develop and implement individualized care plans

• Maintain accurate documentation and case records

• Collaborate with healthcare professionals, caregivers, and community organizations

• Advocate for client rights and access to services

Qualifications:

• Master’s degree in social work or related field

• Current state licensure (if applicable)

• Strong communication and interpersonal skills

• Ability to handle sensitive situations with professionalism and empathy

• Experience in healthcare, community services, or case management preferred
