General Description:

The Physical Therapist at Evara Health plays a vital role in our multidisciplinary team by assisting patients in developing, recovering, improving, and maintaining the skills needed for daily living and working. They are instrumental in providing specialized care to patients with various musculoskeletal and neuromuscular conditions. The Physical Therapist collaborates with a multidisciplinary team to ensure holistic, patient-centered care, aligning with our mission to deliver quality healthcare services.

About the Role:

  • Patient-Centered Evaluation & Care – Perform comprehensive patient evaluations to identify functional limitations, develop individualized treatment plans, and provide evidence-based therapeutic interventions. Monitor patient progress, modify treatment plans as clinically appropriate, and work with patients and families to promote functional independence, recovery, and long-term wellness.
  • Clinical Collaboration & Teamwork – Partner with physicians, advanced practice providers, nurses, medical assistants, and other members of the care team to coordinate comprehensive, patient-centered care. Communicate clinical findings and recommendations effectively to support continuity of care and achieve the best possible patient outcomes.
  • Patient Education & Engagement – Educate patients and caregivers on treatment plans, therapeutic exercises, injury prevention, home exercise programs, and strategies to maintain progress beyond the clinical setting. Build strong therapeutic relationships that encourage patient participation and accountability throughout the rehabilitation process.
  • Mentorship & Clinical Leadership – Support the development of residents, students, interns, and other trainees through clinical education, mentorship, and hands-on guidance. Serve as a resource to clinical staff and contribute to a culture of learning, collaboration, and continuous improvement.
  • Documentation, Compliance & Quality – Maintain timely, accurate, and comprehensive patient documentation in accordance with clinical, organizational, and regulatory requirements. Ensure compliance with HIPAA, HRSA, state and federal regulations, accreditation standards, and Evara Health policies and procedures.
  • Evidence-Based Practice & Professional Growth – Stay current with advances, emerging research, and best practices in physical therapy. Apply evidence-based approaches to clinical decision-making and actively participate in initiatives that improve quality, efficiency, patient satisfaction, and clinical outcomes.
  • Quality & Operational Excellence – Contribute to the efficient operation of physical therapy services by promoting appropriate utilization of resources, maintaining a safe and welcoming treatment environment, and identifying opportunities to improve workflows, patient access, and the overall patient experience.

This role will be based at Evara Health’s new Physical and Occupational Therapy Center, located at our headquarters at 14100 58th Street N. in Clearwater. This newly developed space is designed to expand access to rehabilitation services for the patients and communities we serve, giving you the opportunity to be part of a growing program from the ground up.

Education and Experience:

  • Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • A minimum of two years of professional experience in physical therapy, demonstrating a broad range of clinical skills in a healthcare setting.
  • Exceptional communication and interpersonal skills to effectively engage with patients, families, and healthcare teams.
Licenses, Certification or Registration:
  • Valid and active state licensure as an Physical Therapist required.
  • Basic Life Support (BLS) certification required.
