Schedule: Variable Shift | 4×10s | Negotiable
Shift Options: 6:00 AM–4:20 PM | 8:00 AM–6:30 PM | 10:30 AM–9:00 PM

Job Summary:
Supports medication reconciliation and pharmacy operations, helping ensure accurate medication information throughout the patient’s hospital stay and at discharge.
Key Responsibilities:
Reconcile home medications with inpatient medication orders.
Assist with medication reconciliation for high-risk patients at discharge.
Identify and communicate medication discrepancies to providers.
Collect and analyze medication reconciliation data.
Support pharmacy staff under the direction of pharmacy leadership.
Requirements:
State Pharmacy Technician License – Required (CA/OR)
IV Experience – Required
Epic Experience – Required
Pharmacy Technician Board Certification – Preferred
3+ years of hospital pharmacy experience – Preferred
