Nurse Practitioner /Physician Assistant-Skilled Nursing Facility

We are hiring a Board-Certified Nurse Practitioner or Physician Assistant to join a collaborative long-term care team providing high-quality care to both short- and long-term residents. Must have skilled Nursing Facility experience.

Key Responsibilities:

• Provide primary care services, patient assessments, and treatment planning
• Collaborate closely with physicians and nursing staff in a team-based environment
• Build strong relationships with residents and their families
• Maintain accurate and timely clinical documentation
• Ensure compliance with healthcare regulations and best practices

Qualifications:

• Board-Certified Nurse Practitioner or Physician Assistant
• Strong clinical, communication, and teamwork skills
• Experience in long-term care or hospital settings preferred. We welcome new graduate Nurse Practitioners and Physician Assistants who are enthusiastic about geriatric medicine and committed to caring for older adults!
• Commitment to patient-centered, evidence-based care.
• Must have skilled Nursing Facility experience.
• Extensive training is provided.
