About the Job

Provide skilled nursing and personal care according to the client’s individualized care plan.
Perform ventilator, tracheostomy, and G-tube care.
Monitor and document the client’s condition and report changes promptly.
Administer medications and tube feedings as ordered (RN/LPN).
Maintain accurate documentation.
Follow infection control and safety protocols.
Communicate effectively with families and the healthcare team.
