Qualifications and description for a Social Worker LSW / CSW

• PA Social worker CSW / LSW License

• Conduct Social Service assessments and documents according to state and federal regulations

• Provide Psychosocial assessments and relevant parts of Minimum Data Set (MDS)

• Counseling Residents

• Gerontology / Nursing Home Experience preferred
