Social Worker

Cayuga Nursing & Rehab is recruiting for a full-time Social Worker.

Job duties

Providing social services for residents

Acting as a liaison between medical staff, families, and outside agencies.

Interviews residents and families for social histories

Ensures that social and personal aspects of resident care are properly managed Collects and updates information on community health and welfare resources

Assist with admissions and discharges

Conduct care plan meetings & discharge meetings

Professional Requirements

Bachelor's degree in social work or closely related field required

Experience in long term care preferred (1-2 years minimum)

Familiarity with Point Click Care preferred
