MDS Coordinator

Wayne County Rehab is seeking an MDS Coordinator for our Skilled Nursing Facility.

Duties Include:

• Completing accurate assessments, MDS & care plans as assigned.

• Initiating care plans and supporting activities as assigned.

• Maintaining & updating all care plans and assessments as required.

• Ability to identify problems and to communicate those problems to the DON and staff concerned

• Always Protecting the confidentiality of Resident & Facility information.

Requirements Include:

• Valid NY State RN License

• Long Term Care Experience Required!

• Must be highly organized, professional & motivated

• Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing

• Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans

Job Type: Full-time

Work Location: In person

Pay: $95,000 - $100,000
