About the Role

As a Social Services Assistant at Spring Creek Rehab, you will:

• Provide emotional support and counseling to patients and their families, addressing their social and emotional needs

• Conduct assessments to identify patients' social and emotional needs, and develop personalized plans to address these needs

• Facilitate communication between patients, families, and healthcare providers to ensure coordinated care

• Coordinate with community resources and local organizations to provide patients with access to necessary services and support

• Maintain accurate records and reports of patient interactions and progress

Requirements

• 1 year of experience in social services in long term care

• High school diploma or equivalent required; degree in social work, counseling, or a related field a plus

• Excellent communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment and prioritize tasks

• Strong empathy and emotional intelligence

• Ability to maintain confidentiality and handle sensitive information
