Position Type: Full-Time
Pay: Based on Experience
Neffsville Nursing and Rehabilitation is seeking a dedicated and detail-oriented RNAC Director to oversee the resident assessment process and ensure accurate, timely, and compliant completion of MDS assessments, care planning, and related documentation.
This full-time position is ideal for an experienced MDS professional who is committed to quality resident care, clinical outcomes, and regulatory compliance.
ResponsibilitiesOversee and coordinate the facility's MDS assessment process
Conduct resident assessments according to established schedules and in response to changes in condition, hospital readmissions, and other significant clinical events
Complete MDS assessments, Care Area Assessments (CAAs), care plans, and supporting documentation in compliance with federal and state regulations
Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team
Coordinate with Nursing, Therapy, Social Services, Dietary, Activities, and other departments to ensure timely completion of assessments and care planning
Accurately observe, assess, and communicate changes in resident condition
Prepare and maintain the MDS schedule and ensure timely transmission of MDS data
Monitor documentation for accuracy and regulatory compliance
Support facility quality measures, audits, surveys, and reimbursement initiatives
Collaborate with facility leadership and the interdisciplinary team to promote quality resident outcomes
Serve as the facility's Infection Preventionist (IP), assisting with infection surveillance, prevention initiatives, staff education, and regulatory compliance
Provide Wound Nurse services, including wound assessments, treatment coordination, documentation, physician communication, and monitoring resident progress, as applicable
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania
Minimum of one (1) year of prior MDS/RNAC experience in a skilled nursing or long-term care setting
Working knowledge of MDS processes, RAI guidelines, CAAs, care planning, and Medicare/Medicaid reimbursement
Strong understanding of skilled nursing regulatory requirements
Experience with Infection Prevention and/or wound care preferred
Strong organizational, communication, leadership, and time management skills
Ability to work independently while collaborating effectively with the interdisciplinary team
