Forest Hills Place is looking for a Resident Care Coordinator to join our Amazing Team!
The Resident Care Coordinator assumes delegated responsibility and accountability for the provision of resident care according to professional standards of nursing practice in compliance with state and federal laws and regulations.
Leadership and Mangement -The Resident Care Coordinator is responsible for ensuring that all staff members are trained and motivated to deliver the highest level of care to eash resident.The RCC ensures that the team is aligned with Forest Hills Place core values includes ensuring that all task are completed in a timely manner, working collaboratively with otherdards team members,and ensuring that all tasks are completed in a safe manner. Oversee and manage all aspects of the nursing department, ensuring high-quality ptient care and adherence to regulatory standards.
Facilitate excellent relationships through open channels of communication with residents, families,and staff.
Responsibilities:
- Must have expericing working with seniors(age 65+) or with long term care programs, including knowledge of patient care, wellness practices, health education, patient safety issues, as well as ability to develop senior- specific programs/ activities for which you must be able to provide supervision as needed.
- Must be able to work a flexible schedule including weekends and holidays as needed.
- Prefer if have an clear understandng of operation of the McGregor PACE program and Medicade Wavier Program.
- Can utilize Microsoft office Programs , Point Click Care.
- Works with Wellness Director to supervise, oversee and coordinate the work of Community Team Members to meet Resident needs.
- Participates in service planning as well as renewal and maintenance of these plans.
- Ivolves being completely familiar with state regulations governing Assisted living and ensuring compliance in department by regulary reviewing regulation checklist.
- Performs timely and accurate resident assesments utilizing assessment tools,and communicating changes to the Executive Director for billing purposes.
- Provides oversight with medication mangement to assure all MAR's are correct and that residents receive medication timely and accurately.
- Promotes the Company's Mission Statement.
- Working with consultant pharmacist as needed to assure community is in compliance.
- Ensures all charting is performed in a timely manner and in accordance with state regulations.
- Provides timely and honest communcation with families in order to promote good relationships and to keepthem informed about residents condition and community events.
- Provides marketing support within the community by participating in tours and discussions with potential residents and their families about clinical needs of the prospect.
- Provide assesments for potential residents to determine eligibility for Assisted Living.
- Creates postive employee relations through staff programs,recognition of excellent performance, and retention though regular feedback systems and promotional incentive programs.
- Provides appropriate guidance for employees by palning work schedules assigning work duties while on shift,and directing the work on a daily basis.
- Provides on-call backup to community when not on duty.
- Maintains confidentiality of all pertinent personal or health information concerning residents and staff.
- Ensures that any incident reports are completed and promptly.
