Position: Home Health RN
Schedule: M–F, 8 AM–5 PM
Hours: 40 hours guaranteed
On-call: Every other week/weekend
License: SC RN
Territory: Florence, Marion, Dillon & Marlboro counties
Mileage: $0.48/mile
Productivity: 30 units
Required Skills: Home health, admissions, ROC, recertification, wound VAC, PICC, wound care, LVAD, trach
Main Duties:
Assess patients and manage/evaluate care plans
Provide case management, health maintenance, and disease prevention
Coordinate care with physicians and other healthcare providers
Manage medications, orders, appointments, and clinical documentation
Monitor patient changes and communicate with physicians
Coordinate hospital admissions/discharges
Supervise LPN/LVN staff as needed
Train staff on health, safety, and patient-specific needs
