The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.

- All general responsibilities of a Staff Pharmacist. \n
- Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. \n
- Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. \n
- Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. \n
- Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. \n
- Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings. \n

- Education: Pharm. D. or BS in Pharmacy \n
- Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years) \n
- Licensure/Certification: \n

- BLS within 30 days of hire (ACLS and/or PALS if code attendance required); \n
- Board Certification in Pharmacotherapy (BCPS) \n
