This is a multi-setting pharmacist position covering Outpatient, Skilled Nursing Facility (SNF), and Inpatient Pharmacy services. The pharmacist will provide medication management, clinical interventions, medication safety, dispensing, patient counseling, and support to physicians and nursing staff.

Key Responsibilities\n

Outpatient Pharmacy

\n
    \n
  • Review, interpret, and dispense prescriptions.
  • \n
  • Counsel patients on medications and proper use.
  • \n
  • Provide drug information to patients and licensed healthcare staff.
  • \n
  • Ensure compliance with California and federal pharmacy regulations.
  • \n
\n

Skilled Nursing Facility (SNF)

\n
    \n
  • Perform monthly Medication Regimen Reviews (MRR).
  • \n
  • Prepare quarterly reports on medication trends and findings.
  • \n
  • Participate in CQI meetings.
  • \n
  • Provide medication safety education to nursing/SNF staff.
  • \n
  • Support the SNF Director of Nursing as needed.
  • \n
Systems & Equipment\n

The pharmacist must become proficient with:

  • CPOE – Evident System
  • \n
  • Pyxis Medication Dispensing Units
  • \n
  • eHR/EMR systems
  • \n
  • Automated Dispensing Units
  • \n
  • IV Laminar Flow Hood
  • \n
  • Medication error reporting systems
  • \n
Clinical Responsibilities\n

A major part of this position is clinical pharmacy practice, including:

  • Clinical interventions and therapy monitoring.
  • \n
  • Patient-specific medication verification.
  • \n
  • Therapeutic drug monitoring.
  • \n
  • Renal function calculations and dose adjustments.
  • \n
  • Reviewing medications for therapeutic appropriateness.
  • \n
  • Preventing medication errors.
  • \n
  • Documenting clinical interventions.
  • \n
Required Qualifications\n

Must Have:

\n
    \n
  • PharmD (Doctor of Pharmacy) degree
  • \n
  • Active California Pharmacist License
  • \n
  • Minimum 1 year of pharmacy experience
  • \n
  • Strong English communication skills
  • \n
  • Basic computer skills
  • \n
  • Ability to work independently and prioritize workload
  • \n
  • Ability to supervise pharmacy support staff
  • \n

This is more than a traditional retail pharmacist position. The candidate needs to be comfortable working across inpatient hospital pharmacy, outpatient pharmacy, and SNF pharmacy services.
