Registered Nurse (RN) – Home Health | West Virginia

URGENT NEED!

We are seeking an experienced Home Health Registered Nurse (RN) to provide compassionate, high-quality care to patients in their homes. This role involves patient assessment, care planning, case management, and coordination with physicians and other healthcare professionals.

Position Details:

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• On-Call: At least 1 weekend per month
• License: Active WV RN License
• Productivity: 30 points per week
• Mileage: $0.54 per mile
• Counties: Monongalia, Preston, Tucker, Grant, and Wetzel

Required Qualifications:

• Home Health experience
• HCHB experience
• OASIS experience
• Active WV RN license

Key Responsibilities:

• Assess patient health and develop, implement, and evaluate individualized care plans
• Provide health maintenance, disease prevention, and case management
• Monitor patient conditions and communicate changes to physicians
• Coordinate care with interdisciplinary healthcare teams
• Manage medications, physician orders, and required documentation
• Complete nursing assessments, notes, and other required reports
• Coordinate and follow up on medical appointments
• Communicate with hospitals and discharge coordinators when patients are hospitalized
• Supervise LPN/LVN staff as needed
• Train new employees and provide health and safety education
• Ensure patients receive safe, effective, and timely care in the home setting
