Job Description
Position: Home Health / Case Management RN
Schedule: Day shift, M–F
Hours: 40 hours guaranteed
On-call: None
License: WV RN or compact
Territory: Wood, Wirt & Calhoun counties
Mileage: $0.49/mile
Productivity: Minimum 30 points/week
Required Skills: Home health, OASIS, HCHB
Main Duties:
Assess and manage high-risk patients
Develop and monitor individualized care plans
Coordinate care with physicians, social workers, and other providers
Arrange support services such as transportation and housing
Coordinate hospital/care transitions
Monitor patient outcomes and prevent complications
Promote health and disease prevention
Maintain detailed clinical documentation and advocate for patients
Requirements: ADN/BSN, unrestricted RN license, strong clinical judgment, and case management certification.
