POSITION SUMMARY:

 

Reporting to the PRP Manager, the Rehabilitation Specialist assists with developing/facilitating groups/skill building modules to support individuals with mental illness in their recovery. The rehab specialist works in a

group setting coaching and teaching strategies as well as assisting with recreational outings to integrate

individuals in their community.

 

ESSENTIAL RESPONSIBILITIES:

 

• Participate in the development and implementation of skill-building modules.

• Be knowledgeable of person-centered practices

• Participate in the development and implementation of the individualized rehabilitation/ service plan;

provide direct rehabilitative and clinical services to individuals with disabilities; implement an

intervention, coaching, and teaching strategies in accordance with the plan, the program, and the needs

of the individual; provide services at multiple locations throughout the community.

• Identify crisis or potential crises and intervene to facilitate the best possible outcome; maintain calm

demeanor throughout therapeutic interventions; utilize behavior principles, strategies, and techniques as

needed in accordance with Humanim training; enforce and abide by program rules, policies, and

procedures.

• Assist with planning and coordination of activities and outings based on individual needs/plans; for

individuals with short-term service plans, assist with developing and implementing effective

individualized transition plans in preparation for exiting the program to independence.

• Monitor medications in accordance with Humanim protocols if necessary and if required by the

department; maintain, protect, and share clinical knowledge of assigned clients as appropriate.

• Complete all rehabilitation documentation in accordance with program requirements: daily log or

contact note; monthly progress report; IP/IRP reviews, and assessments; maintain all program

operations and consumer records in compliance with state and federal regulations as directed by the

program; complete incident reports as needed within 24 hours of occurrence; complete/submit all

personnel/ agency documentation within required timeframes.

• Maintain communications with the treatment team – internally and within the provider community;

participate in clinical team meetings as needed; document contacts as necessary; Notify supervisor and

treatment team members of changes in clients’ status promptly; Contact on-call supervisor in case of

urgent or emergencies after hours.

• Work as part of a multi-disciplinary team; coordinate and facilitate visits and meetings, as well as report

progress and exchange information in accordance with program requirements.

• Attend supervision meetings as requested; attend and participate in all program and agency

meetings/training/activities.

• Maintain an overall attendance record of greater than 97%, measured over a minimum of a year; provide

proper notice of necessary lateness or absence; follow procedures for requesting time off.

• Maintain positive, professional work behavior with supervisory staff, co-workers, consumers, families,

funders, employers, and the community; convey a professional image when representing Humanim at

the agency and in the community.

• Prepare for and attend meetings on time; successfully complete all required training on time; follow all

departmental and agency policies and procedures; maintain all emergency and safety requirements.

• Maintain and provide verification of valid driver's license and automobile insurance annually; maintain

a safe driving record with an accumulation of fewer than five points; use personal vehicle to transport

clients to necessary appointments/programs/recreational activities, and other destinations as appropriate;

Use a personal vehicle to shadow clients on agency transportation if necessary.

• Provide rehabilitative interventions to promote clients’ use of a variety of transportation resources and

adhere to all procedures for use of HUMANIM vans.

• Other Duties as assigned.

 

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

 

This position must have the ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations. While performing the duties of this position the employee will constantly operate a computer and other office productivity equipment. This position requires standing, moving, walking, sitting, reaching with hands and arms, stooping, kneeling, crouching, and the ability to drive agency vehicles. Frequently may be required to move/lift to 20 pounds. QUALIFICATIONS:

 

• High School diploma required.

• Use of personal vehicle and a valid Maryland driver’s license required.

• Minimum of 2 years of driving experience required.

• Maintain valid automobile insurance required.

• Ability to relate well to staff and board members of different races, ethnicities, genders, sexual

orientations, socioeconomic backgrounds, and religions.
