Home Health Aide Needed

About who needs care: I have congestive heart failure. I've got 5 stents, have had 5 bypasses. I have lamenectomy with fusion c3-c7. I have crps in left leg. My spine is narrowing at base between that and the crps I'm a fall risk. I have copd and I'm type 2 diabetic I have been falling more frequently lately. If I'm outside our apartment I use a walker but inside I use an electric wheelchair. I need help getting to and from bathroom to chair for most part. About the care needs: This will be hard to do but ( from my Dr) if I fall nobody should try to help me to stay upright, just let me fall Services needed include: companionship, and mobility assistance.
