???? Your Core Responsibilities:
* Conduct comprehensive in-home physical or occupational therapy assessments
* Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs)
* Provide hands-on therapeutic interventions and progress monitoring
* Educate clients, families, and caregivers on safe transfer techniques and home modifications
* Maintain clear, reliable communication with families and our clinical coordination team

✔️ What We Are Looking For:
* Active Massachusetts Physical Therapist (PT) or Occupational Therapist (OT) license (Required)
* A compassionate, professional, and dependable clinical work ethic
* Experience in home health, rehabilitative centers, or assisted living environments is preferred
* Strong communication, paperwork management, and teamwork skills
* Reliable transportation to attend scheduled client visits on time
