Responsibilities:
  • Conduct initial assessments of patient symptoms and health concerns via phone or online communication.
  • Triangulate patient information to determine the urgency of care and provide appropriate recommendations.
  • Guide patients on self-care practices and offer education on medical conditions and treatments.
  • Document patient interactions and maintain accurate records in our health management system.
  • Liaison with physicians and other healthcare professionals to coordinate patient care effectively.
  • Collaborate with the healthcare team to ensure optimal patient experience.
  • Follow up with patients to ensure they are receiving necessary care or to gather additional information.
  • Stay current on medical protocols and best practices to provide accurate and effective advice.

Requirements:
  • Current LVN license.
  • Basic life support (BLS) certification.
  • Clinical nursing experience, preferably in triage or a similar role.
  • Strong communication skills, both verbal and written, with an empathetic approach to patient interactions.
  • Ability to work independently and make informed decisions in a fast-paced environment.
  • Attention to detail and strong organizational skills.
  • Proficiency in using electronic health records (EHR).
