Responsibilities:
- Conduct initial assessments of patient symptoms and health concerns via phone or online communication.
- Triangulate patient information to determine the urgency of care and provide appropriate recommendations.
- Guide patients on self-care practices and offer education on medical conditions and treatments.
- Document patient interactions and maintain accurate records in our health management system.
- Liaison with physicians and other healthcare professionals to coordinate patient care effectively.
- Collaborate with the healthcare team to ensure optimal patient experience.
- Follow up with patients to ensure they are receiving necessary care or to gather additional information.
- Stay current on medical protocols and best practices to provide accurate and effective advice.
Requirements:
- Current LVN license.
- Basic life support (BLS) certification.
- Clinical nursing experience, preferably in triage or a similar role.
- Strong communication skills, both verbal and written, with an empathetic approach to patient interactions.
- Ability to work independently and make informed decisions in a fast-paced environment.
- Attention to detail and strong organizational skills.
- Proficiency in using electronic health records (EHR).
