THE POSITION

The County Caseworker 1, Supports Coordinator position with Wayne County MH/ID gives you the chance to make a meaningful impact every day. DESCRIPTION OF WORK

This position focuses on guiding individuals through planning, coordination, and access to vital Behavioral Health and Intellectual Disabilities services. The role blends documentation, advocacy, and field work in a fast-moving environment. As a Supports Coordinator you will perform the following duties:

  • Plan Development: Create treatment plans, life management plans, and Individual Support Plans based on consumer needs
  • Service Coordination: Locate appropriate providers and support enrollment into service programs
  • Record Maintenance: Maintain organized files, records, and documentation for continuity of care
  • Consumer Advocacy: Act as a voice for the consumer to ensure approved and appropriate treatment
  • Field Responsibilities: Travel to homes, agencies, and service sites, including driving both in and out of the county
  • Deadline Management: Complete daily paperwork accurately while meeting strict and frequently changing deadlines

Work Schedule and Additional Information:
  • Full-time employment; 37.5 hours/week.
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch.
    • Employee may have overnight travel or out of County travel for meetings or training.
    • ID Supports Coordinators are not on call. In an emergency may be called out.
  • Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Other Requirements:

  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.

  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
