Human Service Associate

A government entity has an immediate need for experienced case managers to assist with a health services program.

Responsibilities:

  • Provide ongoing intensive case management services to 45+ clients in private homes, assisted living settings, and other community-based environments.
  • Perform in-home and assisted living assessments to evaluate the medical and social needs of participants.
  • Conduct face-to-face interviews with participants, responsible parties, and service providers to assess diagnoses, prescribed medications and treatments, functional, cognitive, behavioral, and overall health status.
  • Evaluate the appropriateness and effectiveness of waiver and non-waiver services.
  • Evaluate, document, and monitor participants' general health and well-being.
  • Review hospital discharge plans.
  • Complete designated program client assessment forms.
  • Pre-authorize waiver services and coordinate service providers.
  • Assign OTP devices to clients without landline phones for providers to use when clocking in for services.
  • Review ISAS (In-home Supports Assurance Systems).
  • Review nurse monitor reports and follow up with nurses to ensure timely visits.
  • Monitor clients' medical conditions and service delivery while advocating on behalf of clients with physicians, insurance companies, and other social service agencies and programs.
  • Document case activity in the client's progress notes.
  • Prepare written client assessment reports.

Qualifications:

  • Associate's degree or higher in Human Services or a related field.
  • Minimum of two years of case management experience and/or experience working with individuals in the aging and disabilities community.
  • Ability to learn and utilize Maryland LTSS (Long Term Services and Supports) for day-to-day operations.
  • Proficiency with Microsoft Word and Access.
  • Strong client assessment skills.
  • Excellent verbal and written communication skills.
  • Ability to read and interpret regulations.
  • Knowledge of medical terminology.
  • Valid driver's license, reliable personal vehicle, and good driving record required.

Additional Information:

  • This is a long-term contractual assignment.
  • Full-time schedule: Monday through Friday, 8:30 a.m. to 4:30 p.m. (35-hour work week).
  • Regular travel throughout Baltimore County is required to visit clients in their homes, assisted living facilities, and Medical Adult Day Programs.
  • Mileage reimbursement is provided for work-related travel.
  • Upon successful completion of training and the probationary period, contractual staff may be eligible for up to two days of telework per week.
