As a Full-Time Care Navigator, you become the steady, trusted voice for up to 500 patients each month. Your outreach helps patients better manage chronic conditions, understand preventive care, and stay engaged in their health goals.
Over 75% of your day is spent speaking directly with patients, offering coordination, compassion, and clarity.
This is not a job where you wait for the phone to ring—you drive the connection.
- Make up to 20 outbound patient calls daily
- Conduct assessments within your CMA scope to understand each patient’s unique needs
- Collaborate on personalized care plans
- Serve as the communication bridge between patients, families, and providers
- Educate patients on their conditions, treatment plans, and preventive care
- Track progress and care plans
- Document interactions and work within multiple healthcare systems and tools
- Active CMA
- 5+ years of hands‑on patient care experience with at least 1 year of CCM or RPM experience required
- A dedicated HIPAA‑compliant home office with secure high‑speed internet
- Experience in patient care coordination or case management
- Strong clinical knowledge and a heart for patient engagement
- The ability to work reliably Monday–Friday without shifting schedules
- Associates degree
- Care coordination or case management certification
- Experience with diverse patient populations
- Comfort with various EHR systems and healthcare software
