Job Description

• Evaluate and rehabilitate patients with communication, cognitive, and swallowing disorders.
• Develop individualized plans of care based on patient needs and clinical standards.
• Use standardized assessments and evidence-based practices.
• Provide skilled speech, language, cognitive, and swallowing interventions.
• Monitor patient progress and modify treatment plans as needed.
• Evaluate treatment outcomes and make referrals when appropriate.
• Provide education, consultation, and counseling to patients, families, caregivers, and healthcare providers.
• Communicate patient progress, barriers, and treatment plans with the interdisciplinary team.
• Complete discharge summaries and maintain accurate electronic documentation.
• Record treatment minutes according to regulatory requirements.
• Participate in patient care conferences, utilization review, and rehabilitation meetings.
• Participate in required facility/company training and in-services.
• Obtain necessary DME or supplies related to patient care.
• Supervise and support CFY or student Speech-Language Pathologists when applicable.
• Follow ASHA standards, state practice acts, professional ethics, and facility policies.

Required Experience

• Minimum 1 year of SLP experience – Required
• LTC or Rehab experience – Required
• 3 years holding active CCS – Preferred

Required Licensure & Certifications

• Active Texas SLP License – Required
• CCC-certified SLP – Required
• Active ASHA certification/membership – Required
• BLS – Required

Submission Requirements

• 1 professional reference may be submitted at the time of submission.
• Candidate must accept the posted rate of $88/hour.
• Candidates requesting a higher rate may be redirected.

Key Candidate Requirements

Must have:

✅ Texas SLP license
