Description
American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of recent home health experience and HCHB EMR experience.
Details
- Home health setting covering Snohomish and Skagit County (Mount Vernon) and Island territory
- Territory extends up to a 60-mile radius from the hub office
- Day shift schedule, 8-hour shifts, Monday through Friday, 8:00 AM to 5:00 PM
- No weekend or on-call requirements
- Patient population primarily geriatric
- Average caseload of 5-7 patients per day with 6 visits daily
- Productivity requirement of a minimum of 30 points weekly
- Uses HCHB EMR system
- Requires in-office visits weekly or biweekly for team meetings and supply/admission packet pickup
Requirements
- Minimum 1 year of recent home health physical therapy experience required
- BLS certification required
- OASIS experience required
- HCHB EMR experience required
- Experience with geriatric patient population
- Must be flexible with territory assignments
- Timely documentation and communication required
- License required if applicable
Additional Information
- Main responsibilities include home exercise program development and patient/family education
- Scrubs required in any color; no jeans permitted
- iPad provided for documentation
- Mileage reimbursed at the IRS standard rate
- First-time travelers will not be considered for this position
- Local candidates accepted at the travel rate
