Job Description
We are seeking an experienced Registered Nurse for a Home Health position. The RN will provide skilled nursing care to adult patients in their homes, including start-of-care visits, recertifications, daily visits, and case management.
The RN will manage approximately 30 patients and is expected to complete approximately 6.5 points per day. Weekend and holiday rotation may be required. No on-call responsibilities are required.

Requirements
- Active RN license in the state of Kentucky required at the time of submission
- Minimum 2 years of RN experience preferred
- Minimum 1 year of MedSurg experience in a hospital setting
- Previous Home Health experience required
- Homecare Homebase (HCHB) experience required
- BLS certification from AHA required
- Must have experience with Start of Care, recertifications, and daily visits
- Must be comfortable with adult patients
- BSN not required
