Responsibilities:

• Provide emotional support and guidance to patients and their loved ones

• Collaborate with the medical team to develop strategies to address patient needs

• Develop and implement patient-centered care plans

• Conduct home visits and provide education and support to patients and families

• Maintain accurate and timely records of patient interactions and care plans

Requirements:

• Master's degree in Social Work (MSW) or related field

• License to practice as a Social Worker in the state of Texas

• 2 years of experience in a SNF setting

• Strong communication and interpersonal skills
