Job Description

A well-established skilled nursing and rehabilitation center in Rockland County, New York, is seeking a compassionate and experienced Speech-Language Pathologist (SLP) / Speech Therapist to join its rehabilitation team.

The Speech-Language Pathologist will evaluate and treat residents with communication, cognitive, speech, language, and swallowing disorders. The ideal candidate will be committed to helping residents improve their functional abilities, independence, safety, and overall quality of life.

This position offers the opportunity to work collaboratively with an interdisciplinary team in a supportive rehabilitation and long-term care environment.

Key Responsibilities

  • Evaluate residents for speech, language, cognitive-communication, voice, and swallowing disorders.

  • Develop individualized treatment plans based on evaluation findings, physician orders, and resident needs.

  • Provide skilled speech therapy services to short-term rehabilitation and long-term care residents.

  • Evaluate and treat residents with dysphagia and make appropriate recommendations regarding swallowing safety.

  • Recommend appropriate diet textures, liquid consistencies, and swallowing strategies in collaboration with nursing and dietary staff.

  • Provide treatment for residents experiencing communication difficulties related to stroke, neurological conditions, illness, injury, or other medical conditions.

  • Provide cognitive-communication therapy to address memory, problem-solving, attention, orientation, and executive functioning when appropriate.

  • Monitor resident progress and modify treatment plans based on clinical outcomes.

  • Educate residents, families, caregivers, and facility staff regarding communication and swallowing strategies.

  • Maintain accurate and timely evaluations, progress notes, treatment documentation, and discharge summaries.

  • Participate in care planning and interdisciplinary team meetings as appropriate.

  • Collaborate with physicians, nurses, physical therapists, occupational therapists, dietitians, and other healthcare professionals.

  • Ensure therapy services are provided in accordance with professional standards, facility policies, and applicable federal and New York State regulations.

  • Maintain resident confidentiality and adhere to HIPAA requirements.

Qualifications

  • Current New York State license as a Speech-Language Pathologist required.

  • Master's degree in Speech-Language Pathology or Communication Sciences and Disorders from an accredited program.

  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.

  • Previous experience in a skilled nursing, rehabilitation, long-term care, hospital, or geriatric setting preferred.

  • Experience evaluating and treating swallowing disorders and dysphagia strongly preferred.

  • Strong clinical assessment, documentation, and communication skills.

  • Ability to work effectively as part of an interdisciplinary rehabilitation team.

Compensation

$40-$50/hr based on experience and qualifications.
