We are seeking a talented and compassionate Physical Therapist (PT) to join our dynamic team at Rivers Edge Nursing & Rehab in Muscoda, Wisconsin. As a Per Diem Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our residents.

About the Role

As a Physical Therapist at Rivers Edge Nursing & Rehab, you will be responsible for designing and implementing individualized rehabilitation plans to help our residents achieve their maximum potential. Your expertise will focus on assessing and treating residents with a variety of physical impairments, including injuries, illnesses, and disabilities. You will work closely with our interdisciplinary team to ensure that our residents receive comprehensive care that meets their unique needs.

Responsibilities

• Design and implement individualized rehabilitation plans to achieve optimal outcomes for residents
• Conduct thorough assessments of resident physical function and develop goals and objectives
• Provide one-on-one therapy sessions, as well as group therapy sessions when appropriate
• Utilize a variety of techniques and modalities to promote resident healing and well-being
• Collaborate with other healthcare professionals to develop and implement effective treatment plans
• Maintain accurate and timely records of resident progress and therapy sessions
