Full-Time
Salary: Up to $90,000 per year
We are seeking a compassionate and experienced Registered Dietitian (RD) to join our team at a premier Skilled Nursing and Rehabilitation Facility.
The Registered Dietitian will play a vital role in promoting resident health, wellness, and quality of life by assessing nutritional needs, developing individualized care plans, and working closely with the interdisciplinary care team.
ResponsibilitiesConduct comprehensive nutritional assessments for residents.
Develop and implement individualized nutrition care plans.
Monitor residents' nutritional status, weight changes, and dietary needs.
Review and update nutrition interventions as needed.
Collaborate with physicians, nursing, dietary services, speech therapy, and the interdisciplinary team.
Participate in care plan meetings and interdisciplinary team discussions.
Provide nutrition education and guidance to residents, families, and staff.
Ensure compliance with federal, state, and facility regulations.
Maintain accurate and timely clinical documentation.
Monitor therapeutic diets, supplements, and food preferences.
Participate in quality improvement and survey readiness initiatives.
Support infection control, wound healing, weight management, and other clinical nutrition needs.
Registered Dietitian credential required.
Active state licensure or certification, where applicable.
Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, or healthcare setting preferred.
Strong knowledge of clinical nutrition, therapeutic diets, and regulatory requirements.
Excellent assessment, documentation, communication, and organizational skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
