Occupational Therapist (OT)

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our team ! The Occupational Therapist (OT) will be responsible for evaluating, planning, and providing rehabilitative services to residents in order to improve independence, quality of life, and functional abilities in activities of daily living (ADLs).

Key Responsibilities:

• Evaluate residents’ physical, emotional, and cognitive conditions to develop individualized treatment plans.

• Provide direct therapy services, including exercises, activities, and use of adaptive equipment, to restore function and promote independence.

• Educate residents, families, and caregivers on safe techniques and adaptive strategies.

• Collaborate with an interdisciplinary care team, including nursing, physical therapy, and speech therapy, to ensure comprehensive care planning.

• Document all evaluations, progress notes, and discharge summaries in compliance with state, federal, and facility requirements.

• Participate in care plan meetings and contribute to overall resident care goals.

• Stay current with best practices, regulatory standards, and continuing education requirements.

Qualifications:

• Licensed Occupational Therapist (OT) in the state of Wisconsin.

• Graduate of an accredited Occupational Therapy program.

• Skilled nursing facility or geriatric experience preferred, but new graduates are welcome to apply.

• Strong communication, organizational, and interpersonal skills.
