About the Role:
Village Choice Healthcare Inc is looking for a compassionate and dedicated Home Health Registered Nurse to join our growing team in Beaumont, TX. Responsibilities:
- Conduct in-home patient assessments, evaluations, and develop individualized care plans
- Administer medications, wound care, IV therapy, and other skilled nursing treatments
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home safety
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
- Provide compassionate end-of-life and chronic disease management support as needed
Requirements:
- Active RN or LVN license in the state of Texas
- Previous home health, hospice, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Valid driver's license and reliable transportation for home visits
- Proficiency with electronic health records (EHR) and clinical documentation
- Ability to work independently and manage a patient caseload efficiently
