The Direct Care Provider plays a vital role in supporting individuals by assisting with daily personal care tasks, monitoring behaviors, and ensuring their overall well-being. This position involves working closely within a small to medium-sized team and reporting directly to a supervisor. The provider also facilitates access to programs and appointments, promoting client independence and health through compassionate care and effective communication.

Responsibilities

• Provide personal care assistance to clients
• Assist with medication administration
• Monitor and document client behavior and progress
• Prepare meals and support nutritional needs
• Perform housekeeping duties to maintain a safe environment
• Transport individuals to programs and appointments
