RN Case Manager | Permanent | Days | LTACH Accepted

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Shift Details: Days (Rotating Weekends): 8:00 AM to 4:30 PM

Responsibilities

• Coordinate patient care plans across multidisciplinary teams in acute and long-term care settings
• Manage a small patient caseload, providing individualized case management
• Utilize electronic health record systems including Epic, Cerner, and Meditech to document and track patient progress
• Advocate for patient needs and ensure continuity of care
• Facilitate communication between patients, families, and healthcare providers
• Review and monitor patient outcomes and adjust care plans as needed
• Ensure compliance with hospital policies and regulatory requirements

Required Qualifications

• 2 years of Recent (within the last 3 years) Acute Care Experience. Looking for ICU, CCU, Med-Surg or LTAC Case Management experience.
• (RN) Registered Nurse license (FL) or compact
• Associate degree or Certified Case manager
