Case Manager RN - | Full-Time | Days | Acute Care

Job Summary:
Seeking an experienced RN Case Manager to coordinate patient care, discharge planning, resource management, and interdisciplinary communication to ensure safe, high-quality patient outcomes.

Requirements:

• Active RN license (or Compact License)
• Associate Degree in Nursing (BSN preferred)
• 2+ years of recent Acute Care experience
• ICU, CCU, Med-Surg, LTAC, or Case Management experience preferred

Compensations:

• Pay: $34.25 – $47.10/hr (based on experience)
• Sign-On Bonus: Case-by-case
• Relocation Assistance: Case-by-case
