RN Case Manager

We’re hiring an Acute Inpatient Case Manager to support patients throughout their hospitalization, with a strong focus on discharge planning, transitions of care, and placement coordination (including rehab facilities). This is an on-site hospital-based contract role.

Position: RN Case Manager (Acute Inpatient)
Location: Morristown, NJ (on-site)
Schedule: 8:00 AM – 4:00 PM (37.5 hours/week)
Assignment Length: 3 months with potential extension/conversion after
Openings: 3
Weekend Rotation: Every 4th or 5th weekend
Holidays: One summer and one winter holiday

Key Responsibilities

• Coordinate patients through the acute care hospitalization process
• Manage all aspects of discharge planning and care transitions
• Perform utilization management and quality screenings
• Coordinate placement planning, including rehabilitation facility placement
• Support case management needs for subacute patient populations

Required Qualifications

• Minimum 2 years of experience in acute inpatient hospital discharge planning
• must have an active RN license in NJ
• Recent hospital experience that includes discharge planning and rehab placement coordination
• Inpatient case management experience (required)
• Experience working with subacute patients (required)
