Intake Coordinator – Home Health
Location: Phoenix, Arizona OR Las Vegas, Nevada

We are currently seeking an experienced, detail-oriented Intake Coordinator to support our Phoenix, Arizona and Las Vegas, Nevada home health operations. This position may be based out of either our Phoenix or Las Vegas office and will provide intake support for both markets.

Why This Role MattersAs an Intake Coordinator, you will be one of the first points of contact in a patient’s home health journey. Supporting both our Phoenix and Las Vegas teams, you will play an important role in ensuring referrals are handled efficiently and patients receive the care, services, and support they need — when they need it.
You will serve as a key connection between hospitals, physicians, referral sources, patients and families, insurance providers, and our clinical care teams across both markets.
What You’ll DoManage the home health intake and referral process from start to finish for Phoenix and Las Vegas
Collect, review, and organize patient medical, insurance, and demographic information
Coordinate with hospitals, clinics, physicians, case managers, and other referral sources
Help ensure timely and seamless transitions into home health care
Assess patient needs and coordinate appropriate services, treatments, and equipment
Verify insurance coverage and assist with obtaining required authorizations
Maintain accurate electronic medical records and ensure documentation is complete and compliant
Communicate closely with clinical and operational teams in both Phoenix and Las Vegas
Track referrals and follow up on outstanding documentation or information to help prevent delays in care
Serve as a knowledgeable and compassionate resource for patients and families throughout the intake process
Required: Previous home health experience
Strong understanding of medical terminology and home health intake processes
Experience working with electronic medical record (EMR) systems
WellSky/Kinnser experience preferred
Excellent attention to detail and organizational skills
Strong communication skills with patients, families, providers, referral sources, and internal teams
Ability to effectively manage referrals and priorities across two markets
Ability to work independently while contributing to a supportive, collaborative team
Flexible, adaptable, and comfortable working in a fast-paced healthcare environment
Associate’s or Bachelor’s degree in a healthcare-related field
Experience with insurance verification and authorizations
Bilingual skills are a plus
