Home Health Aide (HHA) – Home Health (Field Nurse)  (Hesperia area, CA)

homes assisting them to achieve the highest level of potential in their day-to-day self-care
activities.

Job Description:
- Provide direct patient care in a home setting, ensuring the comfort and well-being of
patients
- Assit patients with personal care (bathing, grooming, dressing)
- Help with mobility and transfers
- Evaluate home environment for hazards or barriers to more independent living.
- Monitor vital signs and report changes to nurse/ supervisor
- Educate and instruct patients, family members, or other patient representatives, in safety
- Accurately document observations, interventions, and evaluations pertaining to patient
care management and services provided.

Requirements:
- Valid Home Health Aide (HHA) in the state of California
- Minimum 1 year of experience as an HHA in Home Health
 - Ability to lift/move patients safely 
- Proficient in taking and interpreting vital signs
