Registered Nurse (RN) – Community-Based Care
Location: Peoria, IL 61601
Schedule: Full-Time
Salary: $85,000+ annually; negotiable based on experience
Position Overview
Avento Health is recruiting a Registered Nurse for a full-time, community-based nursing opportunity in Peoria, Illinois. This role is well suited for an RN who enjoys combining clinical nursing with medication oversight, care coordination, staff education, and health advocacy outside of a traditional hospital bedside environment.
The Registered Nurse will provide nursing care and health support services for adults with intellectual and developmental disabilities while serving as a clinical resource to direct-care staff. The RN will collaborate with healthcare providers, pharmacies, families, guardians, and other members of the care team to promote safe and consistent care.
Key Responsibilities
- Provide nursing care and health support services in accordance with professional standards and applicable regulatory requirements.
- Assess individuals when illness, injury, or changes in physical or mental condition occur and provide appropriate nursing guidance.
- Consult with direct-care staff regarding medical concerns, including on-call consultation as required.
- Train and oversee direct-care staff in medication administration and individualized health-support needs.
- Provide education regarding medical conditions, specialized diets, choking risks, adaptive medical equipment, seizure disorders, and other health needs.
- Coordinate medical, laboratory, health, and dental appointments and related follow-up needs.
- Administer scheduled and PRN medications when needed in accordance with written physician orders.
- Review physician orders and medication records for accuracy and completeness.
- Coordinate medication orders and prescription needs with pharmacies and healthcare providers.
- Monitor for changes in physical health, mental status, medication response, and overall condition.
- Complete required nursing assessments, health-risk documentation, nursing notes, and other clinical records accurately and on time.
- Communicate changes in condition to appropriate healthcare providers, care-team members, families, or guardians.
- Support compliance with applicable state, federal, and quality requirements.
- Conduct CPR and First Aid training when assigned and appropriately certified.
- Provide nursing guidance during urgent situations and participate in crisis response and problem-solving as needed.
Qualifications
- Graduate of an accredited nursing program.
- Current Illinois Registered Nurse license.
- Valid driver’s license and current automobile insurance.
- Must possess or be able to obtain an Illinois Registered Nurse-Trainer certificate.
- Strong nursing assessment, clinical judgment, communication, interpersonal, and documentation skills.
- Ability to provide effective education and clinical guidance to direct-care staff.
- Ability to work independently while collaborating with an interdisciplinary care team.
- Ability to travel locally as required by the position.
- Ability to obtain required first aid, CPR, medication-administration, and related competencies within required timeframes.
Preferred Background
Experience in intellectual or developmental disabilities, community nursing, residential services, home health, behavioral health, case management, care coordination, long-term care, rehabilitation, chronic disease management, medication oversight, or staff education may be especially relevant.
Schedule & Work Environment
This is a full-time position with scheduled hours based on program needs. The RN may provide on-call consultation and respond to emergency situations as required. Work may take place across a variety of community-based settings.
The position may require extended periods of sitting and infrequent lifting or moving of objects weighing up to 25 pounds. The work environment may occasionally involve interaction with individuals experiencing aggressive, threatening, or substance-related behaviors.
Compensation & Benefits
The anticipated salary is $85,000 annually, but compensation is negotiable and may be higher based on experience and qualifications. Candidates who are already qualified Illinois DHS Nurse-Trainers may be especially eligible for compensation above $85,000.
