Manager, Continuity of Care (Case Management)

HCT seeks an experienced nursing professional to serve as the Manager of Continuity of Care for a 270+ bed hospital in California. Responsibilities include, but are not limited to:

• Provide hands-on leadership for a hospital case management department. 
• Ensure case management policies and processes are consistently followed, educate staff on effective case management practices, improve multidisciplinary rounding functions, strengthen staff accountability, and support departmental operational excellence.
• Oversight to include discharge planning, utilization management, length of stay management, avoidable-day and discharge-barrier reduction.
• Establish outstanding rapport and coordination with team, staff, and physicians.

Requirements:

• Education: BSN degree required.
• Experience: Minimum 5 years progressive case management leadership experience in an acute care hospital is required.
• Licensure: Current CA RN license required.
• Certifications: BLS through AHA required or completed during onboarding.
• Computer Skills: Epic
