WCS Healthcare has an immediate full-time opening for a Registered Nurse (RN) in Brooklyn, NY, supporting a Crisis Respite/Transitional Program, Intensive Case Management (ICM), and Critical Time Intervention (CTI) program within a large, well-known social services agency serving individuals with intellectual and developmental disabilities.
The RN will provide comprehensive nursing care to individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI). This role combines skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN will collaborate with interdisciplinary teams to promote health, safety, dignity, and successful community integration while ensuring complex medical, behavioral, and psychiatric needs are addressed during the transition from inpatient care to the community.
Essential Responsibilities
Clinical Care
- Participate as an active member of the intake team and screen individuals being considered for admission to the Crisis Respite/Transitional Program.
- Conduct initial and ongoing nursing assessments for individuals admitted to the program.
- Oversee self-medication administration programs, monitor therapeutic responses, and document outcomes.
- Provide direct nursing interventions, including wound care, diabetes management, respiratory illness support, and chronic disease management.
- Monitor for early signs of medical or psychiatric crises and initiate appropriate interventions.
- Regularly review medication administration records and health monitoring logs, including vital signs, glucose readings, weight, elimination, menses, and other person-specific health information.
- Report significant medical or clinical concerns to appropriate providers and team members, including medication orders, prescription renewals, and medication-related information.
- Monitor and manage medication storage and disposal procedures in accordance with federal and state regulations.
- Develop, monitor, and update individualized health goals and nursing plans based on the current needs of each individual.
- Monitor infection control practices and ensure compliance with OSHA requirements, universal precautions, and applicable health and safety guidelines.
- Order and maintain necessary first aid and medical supplies.
- Provide telephone availability for consultation and basic nursing support as required.
Case Management & Transitional Support
- Partner with ICM teams to address medical and behavioral health needs through comprehensive service planning.
- Provide health education and coaching to individuals, families, and direct support professionals to promote independence and wellness.
- Support hospital discharge planning and community reintegration through Critical Time Intervention (CTI), ensuring continuity of medical, psychiatric, and social services.
- Coordinate follow-up care with primary care providers, specialists, hospitals, and community-based resources.
Collaboration & Training
- Serve as the clinical liaison between healthcare providers, families, and program staff.
- Participate in interdisciplinary treatment planning, case conferences, and care coordination meetings.
- Train and mentor Direct Support Professionals (DSPs) on health-related tasks and emergency response.
- Maintain accurate and timely clinical documentation in compliance with OPWDD, OMH, and DOH regulations.
Requirements
- Current, unrestricted New York State RN license.
- Minimum 2 years of nursing experience.
- Experience with intellectual and developmental disabilities, behavioral health, crisis care, community health, or care coordination preferred.
- Familiarity with OPWDD, OMH, and DOH regulations and program requirements preferred.
- Strong clinical assessment, crisis intervention, and care coordination skills.
- Excellent communication, organizational, and documentation skills.
- Ability to work independently while effectively collaborating with an interdisciplinary team.
- Valid driver's license preferred, as travel to multiple program sites may be required.
