Nurse Case Manager RN | Permanent | Days

The Nurse Case Manager RN plays a critical role in coordinating patient care by assessing, planning, and facilitating discharge plans to ensure safe and effective transitions from acute care settings. This position requires strong clinical judgment and the ability to collaborate with interdisciplinary teams to support positive patient outcomes during the day shift on a permanent basis.

Available Shift: Day Shift: 8:00 AM – 4:30 PM | rotating weekends

Responsibilities

• Conduct comprehensive patient assessments to develop individualized care and discharge plans.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care delivery. dw
• Advocate for safe and timely patient discharges, considering clinical and social factors.
• Monitor patient progress and adjust care plans as necessary.
• Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
• Educate patients and families about care plans and post-discharge instructions.
• Utilize critical thinking skills to identify potential barriers to discharge and develop solutions.

Required Qualifications

• Current Registered Nurse (RN) license.
• Minimum 2 years of recent acute-care experience within the last 3 years required.
• Associate Degree, nursing certificate, or applicable education.
• Strong critical-thinking and clinical assessment skills.
• Excellent verbal and written communication skills.
• Highly organized and adaptable in dynamic environments.
• Ability to confidently advocate for safe patient discharge plans.
• Strong collaboration skills with physicians and interdisciplinary teams.

Preferred Qualifications

• Experience in ICU, CCU, Med-Surg, or LTAC Case Management.
