\n • EMR: EPIC, Wellsky, Careport/PASSR/Teams, Laptop, Phone (ascom)
Hiring Profile:
\n• Must Have Skills/Experience: discharge planning and care coordination, case management minimum 5 years
\n
\n • Preferred Skillset/Experience: More than 5+ years nursing practice in hospital
\n • Type of Patients on Unit: High acuity, complex, high percentage behavioral health
\n • Personality Fit: flexible, easy going problem solver with calm demeanor and can do attitude
