Pride health is actively hiring a Registered Nurse to join our client's home health (Hospice) program in a growing care team serving patients.

Local Pay: $55.00/hour (W-2)
\n*(If your home location is within 50 miles)

Location: Menominee, MI, 49858; Coverage area: Menominee County.
\n
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Patient Load: 5–6 patients per day
On-Call: Every other weekend + 2–3 weekdays
Mileage: Reimbursed from the first patient visit through the last visit

Job Summary:
\nWe are seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality end-of-life care to patients in their homes and residential facilities throughout Menominee County. The RN will coordinate care with patients, families, physicians, and the interdisciplinary hospice team to ensure comfort, dignity, and personalized care throughout the hospice journey.
\nKey Responsibilities:
\n- \n
- Coordinate end-of-life care for terminally ill patients as the primary liaison between families, physicians, and the hospice care team. \n
- Conduct initial and ongoing physical, psychosocial, and spiritual assessments of patients. \n
- Assess patient needs and manage pain, symptoms, and other end-of-life care needs. \n
- Develop, implement, and update individualized patient care plans based on changing patient conditions. \n
- Manage an assigned hospice patient caseload and provide care in homes and residential facilities. \n
- Educate patients and caregivers regarding disease progression, medications, symptom management, and the grieving process. \n
- Communicate changes in patient condition and care needs to physicians and the hospice medical director. \n
- Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR). \n
- Participate in regular Interdisciplinary Team (IDT) meetings to review patient care plans and modify interventions as needed. \n
- Collaborate with the hospice interdisciplinary team to ensure continuity and quality of care. \n
- Provide compassionate support to patients and families throughout the end-of-life process. \n
Required Qualifications:

- Active Michigan RN License \n
- Current CPR Certification \n
- Strong clinical assessment and patient-care skills \n
- Excellent communication and interpersonal skills \n
- Ability to work independently and manage a patient caseload \n
Preferred Qualifications:
\n- \n
- Prior hospice experience strongly preferred \n
- Experience with home health or community-based nursing \n
- Familiarity with hospice documentation and interdisciplinary care planning \n
- Compassionate, patient-centered approach to end-of-life care \n
