- EMR: Homecare Homebase (HCHB) \n

- Active Colorado RN license or active NLC compact license \n
- BLS required \n
- Valid driver’s license \n
- Reliable transportation and auto insurance \n
- Strong assessment and critical-thinking skills \n
- Ability to work independently and manage a caseload \n
- Home health experience preferred but not required \n
- Comfortable with EMR documentation; HCHB experience preferred \n

- Perform comprehensive patient assessments \n
- Develop and update individualized plans of care \n
- Provide skilled nursing care and interventions \n
- Monitor patient progress and communicate changes to the care team \n
- Educate patients and families regarding medications, treatments, and care plans \n
- Coordinate care with LPNs, therapists, caregivers, and providers \n
- Maintain accurate and timely EMR documentation \n
- Participate in the on-call rotation \n
