Weekend Coverage\n

• 5 hours on one Saturday each month

• Take 5 hours off on one weekday during that same week
\n

Requirements\n

\n
• Current, unrestricted Colorado RN license required
\n
• BLS required
\n
• Bachelor’s Degree in Nursing required; Master’s preferred
\n
• Minimum 3 years of clinical hospital experience
\n
• Minimum 5 years of hospital case management experience
\n
• Case Management certification preferred, or willingness to obtain within 2 years
\n
• Knowledge of utilization management, discharge planning, and continuum of care
\n

Responsibilities\n

\n
• Assess, plan, coordinate, implement, monitor, and evaluate patient care needs
\n
• Collaborate with physicians, patients, families, staff, and payers
\n
• Support appropriate and cost-effective utilization of healthcare resources
\n
• Coordinate discharge planning and continuum-of-care services
\n
• Educate physicians and staff regarding utilization and level-of-care issues
\n
• Maintain clear and concise case management documentation
\n
• Collaborate with interdisciplinary teams and review organizations
\n
