Case Manager RN:
\nFacility: Fort Peck Service Unit
Location: 540 W Dayton St, Wolf Point, MT
Contract: 6+ Months
Shift: 8x5, Monday–Friday
Start Date: ASAP upon clearance
We are seeking an experienced Registered Nurse (RN) – Case Manager to support the Fort Peck Service Unit. The RN Case Manager will be responsible for coordinating and managing healthcare services for eligible patients requiring care outside the facility through the IHS Purchased/Referred Care (PRC) program.
\nThe ideal candidate will have prior IHS PRC experience and a strong understanding of referral coordination, care management, medical documentation, and healthcare resource utilization.
Key Responsibilities:\n- \n
- Manage and coordinate Purchased/Referred Care (PRC) services for eligible patients. \n
- Review patient referrals and medical documentation to determine appropriate care coordination needs. \n
- Coordinate specialty and outside medical services with providers, hospitals, clinics, and other healthcare organizations. \n
- Monitor referrals, appointments, authorizations, and follow-up care. \n
- Ensure appropriate documentation and records are maintained in accordance with IHS policies and procedures. \n
- Communicate with patients, families, healthcare providers, and internal clinical teams regarding referred services. \n
- Assist with identifying appropriate healthcare resources and coordinating continuity of care. \n
- Track patient outcomes and follow up on outstanding referrals and appointments. \n
- Support timely processing and coordination of PRC-related services. \n
- Review medical information and referral documentation for completeness and accuracy. \n
- Maintain patient confidentiality and comply with applicable healthcare regulations and IHS requirements. \n
- Collaborate with multidisciplinary healthcare teams to promote quality, cost-effective patient care. \n
- Provide patient education and assistance regarding referrals and outside healthcare services. \n
- Perform other case management and nursing duties as assigned. \n

- Active Registered Nurse (RN) license, BLS(AHA) \n
- Previous IHS Purchased/Referred Care (PRC) experience – REQUIRED \n
- Experience in RN case management, care coordination, utilization management, or referral management preferred. \n
- Strong knowledge of healthcare referral and authorization processes. \n
- Excellent communication, organization, and documentation skills. \n
- Ability to work independently and manage multiple referrals/cases. \n
- Experience working with diverse patient populations and multidisciplinary healthcare teams. \n
- Must be able to complete facility/IHS credentialing and clearance requirements. \n
8x5 Day Shift – Monday through Friday
40 hours per week

- No weekends expected based on the 8x5 schedule \n
Required Qualifications: Active Registered Nurse (RN) license, BLS(AHA) Previous IHS Purchased/Referred Care (PRC) experience – REQUIRED
