Position Overview
We are seeking a highly skilled and compassionate Wound Care Nurse (RN or LPN) to join our clinical team and lead the delivery of specialized wound care services within our facility. In this role, you will be responsible for assessing, treating, and managing a wide range of complex wounds while driving positive clinical outcomes and enhancing resident quality of life.
As the facility’s wound care expert, you will play a critical role in implementing evidence-based practices, guiding clinical staff, and ensuring compliance with regulatory standards. This position offers the opportunity to make a meaningful impact through hands-on care, clinical leadership, and collaboration focused on achieving optimal healing outcomes.
Key Responsibilities
Perform comprehensive wound assessments upon admission and at regular intervals, accurately identifying wound type, stage, and severity
Deliver routine and advanced wound care treatments, including dressing changes, debridement, compression therapy, and the use of advanced wound care products
Collaborate with the interdisciplinary team to develop, implement, and update individualized care plans based on ongoing wound assessments and clinical needs
Educate residents and families on wound prevention strategies, treatment plans, and the role of nutrition and lifestyle in promoting healing
Maintain accurate and timely documentation of all wound assessments, treatments, and progress within resident health records
Monitor wound healing progress and communicate changes in condition or treatment effectiveness to the care team
Work closely with physicians, nursing staff, dietitians, therapists, and other healthcare professionals to ensure coordinated, comprehensive care
Adhere to strict infection prevention and control protocols to reduce risk and maintain a safe care environment
Provide training and ongoing support to nursing staff on best practices in wound care, including current techniques and product usage
Participate in wound care rounds, audits, and quality improvement initiatives to maintain high standards of care and regulatory compliance
Qualifications
Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of New Mexico, in good standing
Wound Care Certification (WCC) or similar credential preferred; willingness to obtain certification within a specified timeframe if not currently certified
Minimum of two (2) years of nursing experience, with at least one (1) year of experience in wound care or a closely related clinical area
Strong clinical knowledge of wound assessment, staging, and evidence-based treatment modalities
Excellent communication and interpersonal skills with the ability to educate, collaborate, and influence across care teams
Demonstrated commitment to compassionate, resident-centered care and improving clinical outcomes
Strong organizational skills with the ability to manage multiple priorities, track progress, and maintain detailed documentation
