As a Case Manager, you won't spend your day behind a desk. You'll build meaningful relationships with residents and their families, coordinate care, solve problems, and become one of the most trusted members of our leadership team.

What You'll Do
  • Build strong relationships with residents and their families.

  • Coordinate care plans and interdisciplinary services.

  • Conduct resident assessments and routine follow-up visits.

  • Collaborate with nursing, recreation, dietary, housekeeping, and leadership to ensure exceptional resident care.

  • Assist residents and families in navigating services and available resources.

  • Maintain accurate documentation and regulatory compliance.

  • Advocate for residents while promoting their safety, independence, and well-being.

  • Participate in care plan meetings and interdisciplinary team discussions.

  • Help create an environment where every resident feels valued and heard.

What We're Looking For
  • Compassionate, resident-focused professional

  • Excellent communication and problem-solving skills

  • Strong organizational and documentation abilities

  • Experience in Assisted Living, Long-Term Care, Social Services, Case Management, or Healthcare preferred

  • Ability to build trusting relationships with residents, families, and team members
